Register your carrier account.
If you haven’t already, register for an account with your medical plan insurance carrier. When prompted during the registration process, give permission for the carrier to reach out to you with support. Call the carrier directly if you need assistance or have questions about registering or how outreach works.

A carrier representative will reach out to you.
Once you register your account and the insurance carrier receives a claim with your prediabetes or diabetes diagnosis, a representative from the carrier will reach out to you. This specially trained representative—sometimes called a care manager or a case manager—will be your personal advocate to help you understand your diagnosis and the resources and treatment options available to you.

Be supported every step of the way.
Your carrier representative can assist you in accessing care and support. They can answer questions about your coverage, find an endocrinologist and other providers that specialize in diabetes care, and connect you with additional resources to help you manage your condition. If needed, they can also help you access diabetes medication, insulin and diabetes supplies, including a blood glucose monitor, test strips, lancets, insulin, insulin pens, pumps and syringes, and storage/travel solutions for your diabetes supplies.

Answer calls from your insurance carrier.
In addition to the carrier representative, you may receive calls from different specialists at your insurance carrier about the many different health programs available to support you. To receive the most personalized support and take advantage of these opportunities, be sure to answer these calls.
Access the Care You Need
Take advantage of carrier resources and your medical plan coverage to help you take control of your prediabetes or diabetes.

Build a relationship with your primary care physician (PCP).
Even if you are not required to select a PCP in your medical plan, it’s a good idea to have one. Your PCP can help you choose a team of specialty providers to support you including a podiatrist, ophthalmologist and endocrinologist. Work closely with your PCP to create your annual care plan of exams and tests recommended for those with prediabetes or diabetes. See tip below for details about what to include in your annual care plan).

Get your annual checkups and follow-up care.
It is especially important for you to receive your annual checkups and follow-up exams, including tests that measure blood glucose level and kidney functions. Make sure you receive annual foot exams to check for diabetic complications and annual eye exams to help protect your vision from damage that can be caused by diabetes. Annual exams and preventive care are covered 100% in the DXC medical and vision plans, when you use in-network providers.

Your medical plan insurance includes coverage for prescribed medication and insulin as well as prescribed diabetes supplies such as a blood glucose monitor, test strips, lancets, insulin, insulin pens, pumps and syringes. Your carrier representative can help you receive the medication, insulin and supplies you need at the lowest possible cost to you.

Regular exercise—even just walking and moving more throughout your day—helps to control blood sugar levels, improve your body’s responsiveness to insulin, and prevent the onset of diabetes for those with prediabetes. Check with your medical plan carrier about the availability of fitness/coaching programs and discounts. Access free online fitness classes and fitness discounts through Perks at Work and free lifestyle coaching through the DXC LifeManagement EAP. Quit tobacco.
Quitting tobacco significantly reduces the risk of diabetes-related complications, such as nerve damage, and improves your health outcomes. If you are prediabetic, quitting tobacco reduces your chances of developing type 2 diabetes by as much as 30% to 40%. Check with your medical plan carrier about coverage for tobacco cessation treatment and medications. You can also join the Tobacco Cessation Program through the DXC LifeManagement EAP, which gives you access to the Clickotine app, Nicotine Replacement Therapy (NRT) and coaching by licensed tobacco cessation specialists—all at no cost to you. Join a support group.
One of the ways you can stay motivated to reverse prediabetes or help yourself live with diabetes is to join a support group for diabetes education, group therapy for stress and anxiety, or peer-to-peer support. Check with your medical plan carrier or local/national diabetes resources to find out what’s available.

Access caregiver support.
If you are a parent or caregiver, you may need assistance and resources to help you care for your loved ones during your (or their) diabetes journey. DXC’s Caregiver and Parent Support benefit through Torchlight provides a variety of helpful online resources and personalized advice sessions with experts who can help with your specific parenting or caregiving issues, at no cost to you. You can use DXC’s Enhanced Family Supports Program through Bright Horizons to find caregivers to assist you, including babysitters, pet sitters, elder caregivers, housekeepers and more.

Take time away from work.
You may need to take time away from work for appointments, treatments or hospitalizations for yourself or a loved one with diabetes. DXC provides job-protected leave of absence options so you can leave work behind and focus on taking care of yourself and your family. Although these leaves are unpaid, you may have access to disability income protection for your own illness (if you enrolled in Short-Term Disability (STD) coverage). You may also use available sick time and flex vacation for your own illness or to care for a family member, including medical appointments. As soon as you are able, reach out to The Hartford, DXC’s Leaves of Absence administrator, to initiate your leave of absence. It’s also important to discuss your plans with your manager.

A Health Advocate can support your financial well-being by helping you access diabetes medication and supplies at the lowest possible cost to you, verify your medical plan benefits before you seek care, find in-network specialists, and resolve claims and billing issues. They can also schedule appointments and transfer medical records on your behalf.

Consider your financial wellness.
A diabetes diagnosis can lead to unexpected financial issues even when you have medical coverage. Talk to your insurance carrier representative about your options to help you manage your health care costs. You can also access financial well-being resources through DXC’s LifeManagement EAP and employee federal credit unions.

Use your tax-advantaged accounts for health care expenses, including travel and transportation.
If you have a Health Savings Account (HSA) or Health Care Flexible Spending Account (FSA), you can use them to pay eligible health care expenses including deductibles and coinsurance as well as any costs you incur for diabetes supplies and insulin. You can change your HSA contributions anytime during the year.

Access your voluntary benefits.
DXC offers hospitalization insurance and critical illness insurance benefit options to provide extra financial support when you are hospitalized or face critical health issues. These benefits provide lump sum cash benefits that you can use in any way you like—to pay costs not covered by insurance and for other expenses such as your rent/mortgage, car payments, childcare and groceries. Although critical illness insurance does not provide a benefit for diabetes itself, it provides a benefit for covered critical conditions related to diabetes including kidney failure.

Facing a chronic health condition such as diabetes makes many people consider the importance of protecting themselves and their loved ones, financially and legally. Depending on your DXC benefit elections, you have access to will preparation services and assistance with other legal issues, including powers of attorney, as well as a variety of legal resources.
